Start with process thinking
Map the flow from customer need to delivered service: inputs, activities, outputs, owners, resources, risks and measures. This makes handovers and weak points visible.
Use risk and opportunity
Supplier changes, new equipment, competence gaps and changing customer needs can affect quality. Assign an owner, action, deadline and effectiveness check.
Learn from nonconformity
Contain the problem, understand the cause, check whether it exists elsewhere and verify that the corrective action works. Closing a record is not the same as preventing recurrence.

Maintenance, procurement, contractor control, competence and change management affect both quality and safety. One process map can help management see the system as a whole.
